Employee's finger amputated by roller machine

At approximately 5:55 a.m. on October 2, 2001, Employee #1 was operating a "beader" roller beading machine, located in the east part of the manufacturing facility. While placing a steel burner ring (2.75 in. long and 5 in. diameter) in the beading machine with his left gloved hand, he inadvertently actuated the machine. His left hand then got caught in the moving beading rollers causing his left index finger to be amputated. The moving beading rollers were creating a pinch hazard and were not guarded at the point of operation. Employee #1 was taken to U.S. Healthworks Clinic in Ontario, CA. for treatment, then to San Antonio Community Hospital in Upland, and finally to San Gabriel Valley Surgery Center where surgery was performed.
