Investigation Summary
At approximately 10:00 a.m. on May 25, 2005, Employee #1, a welder, and two coworkers were involved in the initial placement of steel decking on the roof. While Employee #1 was leaning on the parapet wall and bending down to place a clamp on the sheet he was standing on, the sheet moved from under him. Employee #1 was not using or wearing a personal fall protection system; as a result, he fell to the concrete slab. Employee #1 sustained a comminuted fracture to his left patellar (knee bone), and a bilateral comminuted radial head fractures. Employee #1 was hospitalized for six days.
