Employee Is Injured While Cleaning Tortilla Cutting Machine

At approximately 9:45 a.m. on May 13, 2005, Employee #1 and a coworker, both sanitation technicians, removed the locks from a flour tortilla die cutter and started the machine to clean it. The coworker was sanitizing the conveyor, and Employee #1 was wiping down and sanitizing the framework of the machine with a cloth. While cleaning the frame of the machine, Employee #1's left hand became caught between a chain and sprocket of the machine. She suffered a partial amputation of two fingers and was transported to Loma Linda University Medical Center, where she was hospitalized. Further investigation revealed that the guards that covered a series of chains and sprockets on the machine had been removed prior to the accident.
