Employee's Hand Crushed by Overturned Forklift

At approximately 11:00 a.m. on September 5, 2001, Employee #1, a supervisor and forklift operator for ProcureNet of Ontario, was unloading rebar from a flatbed trailer in the parking lot of the ProcureNet warehouse at 2330 S. Baker Avenue. While Employee #1 was unloading, the forklift overturned and crushed his right hand. Employee #1 was transported to the Loma Linda University Medical Center in Loma Linda where he was treated and hospitalized in excess of one day.
