Employee dies after fall from ladder

At approximately 5:00 p.m. on May 13, 2002, Employee #1 was on the roof of a patio, masking the windows of a residence in preparation for painting. He set an extension ladder up against the Spanish tiles at the edge of the second-story roof, which was at a right angle to the patio roof. Employee #1 was climbing the ladder to mask the windows when it slid sideways on the patio roof tiles and he fell 8 feet 9.5 inches onto a concrete walkway. He sustained severe head trauma and apparently lost consciousness. Employee #1 was transported to the trauma center at Riverside County Regional Medical Center, where he underwent emergency surgery and was placed on life support. He died at 5:30 p.m. on May 15, 2002. The ladder on the patio roof had not been secured.
