Event: 04/10/2003
One Employee Killed, One Injured When Struck By Bales

At approximately 3:00 p.m. on April 10, 2003, Employees #1 and #2 were standing next to a block of three rows of columns of stacked compressed foam bales that were 13 feet high. Employee #2 was discussing the work with Employee #1 when suddenly two bales from the northeast column of north row fell on Employee #1, striking and killing him. One foam bale from the center or northwest column then fell on Employee #2, injuring him seriously. Employee #2 was hospitalized for his injuries. The compressed foam bales were of irregular and rectangular shape and weighed approximately 500 to 700 pounds. The stack was high and unstable.
