Employee's Finger Amputated by Chain and Sprocket

At approximately 2:00 p.m. on February 1, 2007, Employee #1, a machine operator for a manufacturer and distributor of condiments supplies to the fresh-cut produce industry, was attempting to align a small rolling conveyor with an inclined conveyor to prevent product from falling off. The rolling conveyor, built by the employer, rested on four wheels that were not equipped with locks to prevent the conveyor from walking or moving from its location due to employee traffic and production vibration. In addition, the rolling conveyor was not equipped with handles, so its frame members were usually used to maneuver it. The conveyor's chain drive was powered by a 1/2 HP Dayton DC motor (Model # 2M168D, 1725 RPM). As Employee #1 was pulling the rolling conveyor, his right hand fingers were wrapped around the front guard of a chain and sprocket assembly and into the unguarded back side of the assembly. The tip of his middle finger was partially amputated when it was caught in the nip point between the chain and a sprocket (measured at 35 inches above the floor). He was transported to Corona Industrial Medical Center in Corona, then to Riverside County Regional Medical Center in Moreno Valley.
