Employee Killed by Forklift Backing up

Employee #1 was pulling an order in an aisle section of a commercial distribution and warehousing facility that serves the public. He asked a forklift operator to bring a load down from a rack. Employee #1 was in the aisle way, about 6 feet behind the forklift. The forklift operator backed up the forklift very quickly, hit Employee #1, and drove over his right leg, fracturing it. Employee #1 was taken to the hospital and was hospitalized for 2 days.
