Investigation Summary
At approximately 7:15 a.m. on May 22, 2003, Employee #1 was working as a Maintenance Helper in a pie making facility. The employee was assigned to lubricate the Colborne Series 15 Pie Line the employee at the time of the accident had raised the cover that guarded a series of chain and sprocket drives in the top crust sheeting unit of the Pie Line conveyor while it was running. Holding the tip of the grease gun with his right hand, he applied grease to the lubrication points located near the chains and sprockets. The tip of the grease gun momentarily contacted a moving chain and it pushed his finger between a chain and sprocket, severing the tip of his right index finger. The exposed chains and sprockets were located from 36 to 55 in. above the floor.
