Investigation Summary
At approximately 6:30 a.m. on December 18, 2000, Employee #1 was lubricating the molds on an injection molding machine while the machine was in its normal operation. Employee #1 was using a spray to lubricate the molds. While lubricating, the molds closed on Employee #1's right hand and caused amputation of his right middle finger. Employee #1 was hospitalized there over 24 hours.
