Investigation Summary
At approximately 10:00 p.m. on October 10, 2000, Employee #1 was driving a fork-lift, stand up type Komatsu Electric Model FR-236-2A Serial Number FR63016662001. He was driving with out a load towards a wall on the south side of the warehouse near aisles Number 123 and 124. He tried to stop the fork-lift by lifting his left foot off the pedal but the fork-lift did not stop. His left foot was caught between the wall and body of the fork-lift fracturing his big toe and two adjacent toes. Employee #1 was hospitalized and his toe was eventually amputated.
