Employee's hip injured when struck by shotcrete boom

At approximately 6:30 p.m. on January 12, 1997, Employee #1 was using a CAT 455 loader to extricate a Wagner front end loader stuck at the bottom of a 100 ft long ramp. He attached a 20 ft chain to the cab of his CAT and to the bucket of the stuck loader. Employee #1 stopped when his loader reached the top of the ramp, but the Wagner loader continued moving forward. Its shotcrete boom, which extended 8 ft beyond its bucket, went between the two bars supporting the canopy of the front loader. The boom struck Employee #1, dislocating his hip. He was hospitalized for five days.
