Employees foot his caught between two fork lift trucks

At approximately 11:30 p.m. on July 7, 1999, Employee #1 was operating a forklift and as he attempted to stop, he placed his left foot outside the forklift where it was caught between his forklift and an approaching forklift. Employee #1 was hospitalized with a fractured foot.
