Investigation Summary
On August 31, 1999, Employee #1 climbed out of the operator's seat of a forklift and was in the process of hoisting himself up to the pallet on the forks to realign the pallet. His right finger became caught in a small opening on the fork frame. The opening was approximately 0.25 inches wide. In his haste to remove the trapped finger, Employee #1 sustained an amputation to the tip of his right small finger.
