Investigation Summary
At approximately 3:10 a.m. on February 20, 1997, Employee #1 was standing on a 55 gallon steel drum, loading glass bottles onto a conveyor. He fell approximately 33 1/2 in., suffering serious head and arm injuries. He was transported by paramedics to the Loma Linda University Medical Center for treatment. At the time of the accident, Employee #1 was at a secondary employer's worksite.
