Employee's leg fractured when work platform falls

At approximately 11:05 a.m. on November 18, 1996, Employee #1 was on a work platform elevated 11 ft by a Towmotor fork lift and secured to the mast and tine assembly with a chain. The components of the forklift's mast/truss assembly separated, and the platform and Employee #1 fell to the asphalt. he was hospitalized with a compound fracture of his left leg. The Division proposed the employer be cited for a violation of T8CCR 3328(e) because the forklift mast assembly was not properly secured to minimize the hazards of breakage, loosening, or falling.
