Employee's finger severed while using knife to break ice

Employee #1 was setting up the student canteen store for the brunch serving. The usual routine included the onsite delivery of food and beverage products, the quantity of which was determined by a requisition submitted the day before. On the previous day, the canteen had been short of food and beverages. As a result, Employee #1, in an unusual move, had to go to the central kitchen to pick up extra food and drinks. She was using a knife to break up a bag of compacted ice when the knife slipped and severed her right little finger.
