Employee fractures elbow in fall from moving farm machine

On December 11, 1995, Employee #1 and two coworkers were riding across an agricultural field full of bell peppers in a high speed plastic dispensing machine. Employee #1 and his coworkers were sitting in specially-designed seats mounted at the machine's rear, charged with threading new rolls of plastic onto the dispensers before the existing rolls ran out. Employee #1 fell from the machine and, in an attempt to break his fall, landed on his right elbow and fractured it. Causal factors for the accident could not be established due to conflicting accounts of its circumstances.
