Investigation Summary
At approximately 7:00 a.m. on August 24, 1994, Employee #1, who worked at the La Quinta (CA) Golf and Country Club, was operating a Ford tractor, model 6610. The tractor had an attached hydraulic dump trailer to haul dirt from one location to another on the golf course. Employee #1 was driving along an access road when he apparently lost control and struck a palm tree. He was ejected from the driver's seat and landed in front of the tractor's rear tire. Employee #1 was run over by the tractor and suffered a fractured pelvis, right leg, and ribs. The tractor was equipped with roll-over protection, but the vehicle's seatbelts had been stolen prior to the accident.
