Employee's fingers amputated by table saw

On November 29, 1994, Employee #1 was using a Rockwell 10 in. Unisaw, model 34-466, serial #JH3699 to cut 1 in. strips of laminated plastic in a large open area of a carpenter shop. His right hand struck against the saw blade, amputating four fingers. Based on information provided by Employee #1, a coworker, and members of management, the table saw blade was apparently not guarded during the 1 in. setting for cutting strips.
