Investigation Summary
At approximately 2:00 p.m. on February 1, 1995, Employee #1 was cutting large bus and truck tires on a locally designed and fabricated machine that spirally cuts rubber from the sidewall and tread areas. He was manually maneuvering a section of tire through the machine when his left hand became caught between the rotary knife and the gear that pulls the tire through the point of operation. The first digit of his middle finger was amputated. Employee #1 had been provided by a temporary employment agency. The secondary employer, who directed and supervised Employee #1, was cited for a serious violation of T8CCR 4002(a), because the point of operation of the spiral cutter was unguarded.
