Employee's arm and hand injured in pulley wheel

On August 29, 1994, Employee #1, a maintenance worker, had completed repairs on a machine. When he reached through the pulley wheel to retrieve his screwdriver, the machine started unexpectedly. Employee #1's left hand and arm became entrapped in the pulley and were severely lacerated. He was hospitalized.
