Investigation Summary
On September 6, 2007, an employee was delivering the rented aerial boom lift, JLG E300AJP, Serial Number 0300115247. The employee was located inside the bucket of the boom lift, and was driving it off of the trailer, Dakota Manufacturing Company, Inc., Trail-Eze Trailer, VIN 1DA72C7446C018009, Model TE80HT. The bucket was elevated for travel. As the employee was driving off the trailer and down the ramp, the boom lift's front pothole protection devices or wheels got stuck on the hinge of the trailer or dove tail. The employee turned the wheels other than straight and was able to dislodge the machine by giving the machine power to move. Then, the rear pothole protection devices or wheels got stuck on the hinge. The employee attempted to move the wheels from side to side to dislodge the boom lift. When the machine was dislodged, the machine's wheels were turned and the boom lift drove off the side of the ramp with a maximum 2.5 foot drop. The boom lift stayed in an upright position. The employee was launched from the basket and traveled approximately 10-15 feet in the air and landed on the trailer. He was not wearing any safety restraint. The employee was hospitalized for approximately seven days with injuries that included two broken wrists, injuries to the face, and injuries to the knee.
