Investigation Summary
On October 14, 2005, Employee #1, a contract employee with Paint Boy, Inc., was using a 36 ft extension ladder that was not fully extended to caulk underneath the eaves of a two-story building in an alcove. The ladder was on the ground, leaning against a wall and positioned over a scaffold around the alcove. The ladder (Part Number 63045-08) was ANSI approved, and in good condition. The height from the ground to the bottom of the eave, within the alcove, was measured at about 21 ft. A coworker returned to the building and found Employee #1 lying on the ground. Employee #1 was hospitalized for a major brain injury with memory loss. He underwent surgeries to set a fractured mandible and facial bone, and a shunt was placed in his skull. Employee #1 was interviewed, but did not remember how the accident occurred or anything pertaining to the work being done the day of the accident. A Fire Department representative indicated that, when he arrived at the scene, the ladder was tied to the scaffolding with a wire on the right side, and the ladder had rotated over from left to right.
