Investigation Summary
On April 24, 2008, Employee #1 was using a cutting wheel on a grinder to cut a 4 in. by 4 ft steel tubing. To tested a disc (Norton 7 in. Cutting Wheel), he inserted it into the grinder (Makita 7 in. Disc Grinder, Serial Number GA7001L). As soon as he powered the grinder, the disc broke in half, ejecting a piece that stuck him in his forehead. Employee #1 suffered a deep lacerated wound on his forehead. He was transported to St. Jude Medical Center located in Fullerton, CA, where he was treated for his injuries and then released.
