Investigation Summary

Investigation Nr: 201145034
Event: 12/05/2003
Employee injures thumb while pushing rolling cart

On December 5, 2003, Employee #1 was working as a cell lead in the fabrication and machine shop. He was helping two coworkers transfer 1,700 pounds of 20-foot-long pieces of iron onto a 3-foot-wide by 6-foot-long manually operated rolling cart. The load was not secured as the three employees pushed the loaded cart up an approximate 5 degree grade ramp to the entrance of a building. Employee #1 was pushing the left side of the cart when the load shifted onto his right thumb and his thumb was caught between the cart and the load. He was transported to the hospital where his right thumb was amputated at the first joint.
