Employee Dies after Fall from Roof

On November 15, 2006, Employee #1 was a roofer, working on a roofing job on a tower structure on top of a building. The tower had a step-like configuration with a lower level extending to the sides, and the large base of the tower constituting its lowest level. The roof of the main building was 20 ft above ground level. The distance from the top of the tower to the top of the tower's base was approximately 16 ft. The distance from the top of the tower to the asphalt parking lot was 48 ft. The roof surfaces were flat. Employee #1 was working on the top level of the tower, heating mastic with a torch and attaching flashing to the tower's front edge. Employee #1 was injured when he fell to the parking lot. He was not wearing fall protection. He died on November 28, 2006 as a result of the injuries he sustained in the fall.
