Investigation Summary
On July 03, 2006, Employee #1, a forklift mechanic, decided to inspect the underside of a forklift, he was scheduled to repair. He selected a Baker forklift truck, Model Number FJD-070, Serial Number E-1652-113, from the three forklift trucks parked in the area designated for trucks for the mechanics to use. Employee #1 pulled the forklift truck behind the forklift needing repair, a Nisson Model Number CR6H02A30V, Serial Number CRGH02920092. He maneuvered the Baker's forks underneath the Nisson and lifted the Nisson with the Baker forklift. Employee #1 slid underneath the Nisson forklift to look briefly at the area needing repair, when the Nisson forklift slid down the forks and onto Employee #1's abdomen. Employee #1 was able to call out for help. Another employee and the employer came to his aid. Employee #1 was hospitalized.
