Investigation Summary
On June 26, 2003, Employee #1, a utility and supply driver, was operating a 1-ton crane with a Downs Crane and Hoist attachment clamp, Serial Number 22333, Load 1000 lbs, to move bales of materials and supplies to the roller conveyor in a tissue and personal care product manufacturing facility. As Employee #1 finished moving a bale to the roller conveyor and was returning the equipment to its original position, the clamp pin moved to an axis position and the clamp closed on his left hand. The pin shot out and hit Employee #1 in the forehead. Employee #1 was hospitalized and treated for an open fracture on his left hand and a puncture wound to his forehead.
