Investigation Summary
On February 21, 2005, Employee #1 a helper on a garbage truck (Maxon Front Loader, Model Number LOL-38, Serial Number 2421) was riding on a platform extending from the base of the garbage bin at the front of the truck. He was holding onto a bar next to the garbage bin. It had rained that morning. While the truck was traveling approximately 5 miles per hour, the driver went around a tree located on the right side of the street to avoid hitting it, and the bottom part of the forks (the cross-member reinforcement plate, which is a 3 in. steel bar underneath the forks) apparently struck a protruding utility cover. This caused Employee #1 to fall off the platform, onto the street. He was hospitalized at Placentia Linda Hospital for fractured ribs on his right side, fractured bones on the left side of his face (eye socket and check area), and surgery to place titanium plates in his skull and eye area. The employer reported that there were no apparent mechanical defects noted with the truck involved in the accident.
