Employee Is Struck by Equipment Rack

On April 29, 2005, Employee #1 was working in an area where protective plastic caps were removed of from pipe threads. As an empty pipe rack was being moved to a new location, he ran to the rack to retrieve his knife. The rack then slid off the forklift struck Employee #1's right forearm and abdomen. He spent two days in the hospital for observation only.
