Investigation Summary
On May 15, 2005, Employee #1 was replacing a drilling rig cable on to a pulley. The unit's Kelly bar slipped downward eliminating the cable slack, and caught Employee #1's left middle finger tip. Employee #1 suffered the amputation of his finger tip and was treated for his injury but was not hospitalized.
