Employee Suffers Crushing Injury While Servicing Machine

On March 26, 2005, Employee #1, a production night shift supervisor, was replacing Teflon black protector sleeves on arms and beams of the rotating press on the Biko CSM04 machine. The Biko Turning Press is used for loading, pressing and unloading belts of abrasive paper and cloth. The manufacturer of the machine is Biko Meccanica of Italy. It is a rotating press Type PTP1. The overall dimensions of the machine are 1600-mm by 800-mm. The height is 3,100 mm. The working unit consists of three independent working stations, provided with a belt loading conveyor, front hooking, pressing beams and pressing cylinders. The belts are unloaded in a carrousel provided with 20 dowels. This carrousel has the function to store the produced belts. At the time of the accident, Employee #1 was standing on a ladder to reach one of the three arms of the press when another employee pressed the red alarm reset button on the main control panel, which was out of sight Employee #1's position. This machine operator did not check if anyone was in the machine area before pressing the button. After the button was pressed, the arm rotated to its next position, knocking Employee #1 off the ladder and wedging her chest between the press arm and the carrousel arm. As Employee #1 screamed, her coworkers held her weight up off the floor. Another coworker pushed the carrousel advance button which moved the dowel forward releasing Employee #1 from this predicament. Employee #1 suffered a fractured scapula and a fractured rib, together with contusions, and was hospitalized.
