Investigation Summary
At 5:20 p.m. on June 10, 2005, Employee #1, a slit coil machine operator, was working in the packaging department at the Nexcoil San Diego Coil Center. Employee #1 placed four metal coils, each weighing approximately 1500 kg, and measuring 1 to 2 mm thick and 101.3 mm wide, in an upright vertical position so they could be picked up by a hoist and prepared for packaging. Two of the bundles, weighing approximately 3100 kg, fell over and landed on the back of both of Employee #1's heels. No coil racks were in use to support, stabilize, or secure the heavy metal coils and prevent them from tipping over. Employee #1 was transported by ambulance to Mercy Hospital and hospitalized for his injuries.
