Investigation Summary
At approximately 11:15 a.m. on August 17, 2006, Employees #1 and #2 were working for a residential cabinet manufacturer. Employee #1, an operator, mounted two blades (knives) to the router bit (tool holder) of an ONSRUD (model number W1136A, serial number 10680) pin router machine (number 44), and turned the machine power on. As he was moving the wood material to clear the area around the machine, suddenly the blades became loose and flew off the router bit. Employee #1 was struck in the left fore arm, and Employee #2 was struck in the back of his right hand. Employees #1 and #2 sustained serious lacerations and fractures, and were hospitalized. The blades were not properly set up and firmly secured to the router bit to minimize hazards caused by loosening of the router blades.
