Event: 03/06/2006
Employee's Foot Is Crushed by Skid Loader Bucket

At approximately 2:30 a.m. on March 6, 2006, Employee #1, a foreman cum operator, was preparing to transport a Caterpillar skid steer loader, Model Number 268B. He drove the skid steer loader onto a parked flat bed trailer truck. He got off the loader, left its engine running with the bucket raised, and secured it with a single chain onto the trailer. He then climbed into the loader cab and began lowering the bucket, with his left foot hanging out of the side of the cab. As the bucket came down, it hit his foot, causing a crushing injury of his left foot. Employee #1 was taken to Kaiser Permanente Hospital, where surgery was performed on his foot, and he was hospitalized for six days.
