Employee Crushed Between Forklift and Wall

A fulltime employee was replenishing shelves with stock at a warehouse-distribution facility. He was driving a Raymond Easi standup forklift (Model No. R40TT-M; Serial No. EZ- B-01-21703) in the backward direction. At about 4:45 p.m., he lost control of the vehicle as he was traveling down one of the aisles of miscellaneous dry food items. At the end of the aisle, the forklift continued travelling with the employee and struck a wall. Before impact, the employee's body was partially out of the operator's position, and he was crushed between the forklift body and the wall. Emergency medical services airlifted him to a medical center. However, he died from his injuries at about 5:30 a.m. the next morning.
