Event: 11/15/2006
Employee Is Injured in Fall from Ladder

At approximately 2:05 p.m. on November 15, 2006, Employee #1, a maintenance mechanic, was working from an extension ladder to remove an acoustic tile from the 12-foot-high suspended ceiling. The tile marked for removal had a stain on it, which Employee #1 was also instructed to determine its origin. While Employee #1 was standing on the ladder, about 8 feet from the ground, holding onto the ladder with one hand, and inspecting a water line on the ceiling tiles and adjacent wall, he fell toward his right side. As he fell, Employee #1 struck an adjacent refrigerator and rolled to the tile floor below. Emergency services were contacted and Employee #1 was taken to Loma Linda University Medical Center, where he was hospitalized and underwent surgery to repair a fracture to his left elbow. Additionally, he received treatment for head, neck and hip injuries.
