Employee's Hand Is Injured in Chain and Sprocket Drive

At 6:00 a.m. on July 24, 2002, Employee #1 was cleaning a sprocket chain guard at a vegetable packing plant. A supervisor directed Employee #1 to wipe off the guard at Cull line #3 while the drive was operating. The guard was located 1-in. away from the motor to Cull line #3, and it had an opening (7.5 in. by 4 in.) on the side that was facing the motor, through which the sprocket-chain drive was connected to the motor and arbor. The motor was manufactured by Reliable (Model Tiger 150, 1/2 hp). While Employee #1 was wiping the top of the guard, the end of the rag became caught in the exposed sprocket-chain drive. As the rag was pulled into the chain drive, his right fingers were also pulled in. He was hospitalized for a crushed index finger and an amputated middle finger. There were no eyewitnesses to the incident.
