Investigation Summary
Employee #1, an electrician, was working from an extension ladder while pulling wires. The ladder was placed at a low angle, approximately 2 ft out from a wall, and the concrete floor had recently been sealed. The ladder slipped out from under Employee #1 and he fell approximately 18 ft. Employee #1 suffered a fractured hip and was hospitalized.
