Investigation Summary
On August 28, 2001, an employee working as an assembler was operating a triflow machine, which assembled small Teflon balls into a plastic containment device. One of the balls became stuck in its track and the employee reached to release it, and accidentally activated the control switch of the dispenser. The tip of her left index finger was amputated.
