Investigation Summary
At 12:55 a.m. on August 21, 2017, an employee raised the tension flip up segment of a Baktek incline conveyor. The employee was standing on a stool, and began wiping the belt of an incline conveyor with a washcloth. The employee stretched her left forearm to support a segment that was falling and was struck by it, suffering a left forearm fracture. The employee was admitted to Highland Hospital for treatment and released 11 hours later.
