On January 21, 1997, Employee #1, of Image Systems Acoustics and Wall Coverings, was operating a Powermatic saw, model #66, serial #9466209, when he injured three fingers on his left hand. His middle finger was amputated and his thumb and index fingers were lacerated and permanently disfigured.
