Employee catches left pinky finger in machine

On March 30, 2002, Employee #1, of Calloway Golf, Golf Ball Manufacturing Division, was operating a rubber mixer, which included the zinc oxide hopper. The finished rubber product was for the center of golf balls. Employee #1 was at the zinc oxide hopper (rubber mixer) and just changed the super sack containing zinc oxide powder before his break. After break, he noticed on the computer screen that the powder was low, which was odd, since he had just changed the sack. He decided to check the flow of the powder. He first turned on the massager. The massager flaps up and down to help the zinc oxide powder flow from the sack into the hopper. He then climbed the side of the hopper. He placed his right foot on the rim of the hopper, left foot on the control box, and his left hand on the steel bar so that he could pull himself up above the hopper to visually check the powder flow. As the massager came down against the steel bar, Employee #1's left hand was caught between the massager and the steel bar. He immediately removed his left hand when the massager lifted. Employee #1 sustained an amputation to his left pinky finger and broke his left ring finger.
