Employee's Index Finger Is Amputated in Dough Sheeter

At 7:15 a.m. on March 21, 2006, Employee #1, a foreman was taking the job of the operator on the bread Line at Dough Sheeter Number 1, while the operator was on break. The dough sheeter was listed on the Bimbo Bakery Equipment list, as Number 221, on the Dough Sheeter line Number 1. Employee #1 was adjusting the feed mechanism knob, while standing on the work platform. When he was stepping down, he brushed against the frame of the guard mechanism on the side of the machine. At that time, part of the guard mechanism came loose from the frame of the Sheeter and fell on the floor. At that same time, Employee #1 put out his hand to keep from being hit by the falling machine part. He stuck his hand directly against the newly exposed chain drive mechanism of the conveyor. His hand was caught between the chain and sprocket drive and the first joint of his left index finger was amputated.
