Investigation Summary
At 9:20 p.m. on August 26, 2017, Employee #1 was making an adjustment to the belt of the shear roll #2. The employee removed the guard while the shear roll operated, and attempted to adjust the tracking of the conveyor belt. Employee #1's right hand entered the belt and roller, crushing his hand and causing a severe laceration. The employee was hospitalized for his injury.
