Investigation Summary
At approximately 2:30 p.m. on October 4, 2002, Employee #1, a 47-year-old maintenance mechanic in a paper plate manufacturing plant, was walking in a designated aisle in transit to a another part of the plant. At the same time, a coworker was operating a forklift (Make: Nissan, Model: 30, Serial Number: CRJ-9N-2604), in an adjacent isle, to deliver pallets of corrugated box material. The forklift was equipped with both a back-up alarm and warning lights. Employee #1 walked behind the forklift, which was on his right, and forward several feet from the aisle where the forklift was located. The coworker sounded a backup alarm, but did not look back before moving the vehicle backwards. Although Employee #1 heard this alarm, he did not have a chance to move out of the forklift's path due its excessive speed. The lift struck his right side, causing him to fall on his left side, and did not stop until it was completely within the travel aisle. Employee #1 was hospitalized and required surgery to place pins and screws in the head of his femur.
