Investigation Summary
At approximately 11:15 a.m. on January 8, 2004, Employee #1, a truck driver with Preferred Personnel, was delivering Amway products to a U.S. Post Office in Coronado, CA. He backed his Isuzu truck into the loading dock area in the rear of the post office, but was unable to get the truck flush against the dock due to the position of other vehicles and a pole. While removing boxes from the back of the truck and placing them on the dock, Employee #1 had one foot inside the truck and his other foot on the dock. After losing his footing and slipping on the dock, he fell backwards between the dock and the truck. Employee #1 was hospitalized at Sharp Memorial Hospital in Coronado, CA, for a fractured left ankle and right foot.
