Investigation Summary
At approximately 9:30 a.m. on May 22, 2006, Employee #1 was working alone at the raw material storage yard. Employee #1 was manually transferring Tubing Head Castings, weighing approximately 350 pounds each, from one wooden pallet to another for transport to the Machine Shop. Employee #1 tilted the Tubing Head Casting and was attempting to roll it onto the receiving pallet when it started to fall, pulling employee off balance and causing him to fall from the pallet surface which is approximately 6 in. above ground level. Employee #1 landed in an awkward position and resulting in a fracture to Employee #1's left knee and lower leg. Employee #1 was hospitalized.
