Investigation Summary
On September 10, 2004, Employee #1 was working in the thermal oxidizer and control room areas of an Acrolein plant when he suffered from overexposure to Acrolein vapors from a leak. He suffered respiratory injuries but was not aware of the seriousness of his injuries and did not report them until December 27, 2004. He received initial treatment on December 28, 2004 at Bakersfield Occupational Medical Center and was diagnosed as suffering from chemical exposure pneumonitis. The leak had been identified on September 7, 2004 but the plant continued to operate until the source of the leak was identified on September 10, 2004.
